Job summary

Destiny Springs Hospital in Surprise, AZ is seeking an experienced RN intake coordinator who will work under the direction of a registered nurse to support care plans and patient flow.

You will assess patient needs, coordinate with the health care team, document in the patient record, and assist with admission processes while ensuring compliance with policies and delivery of high-quality patient care.

Qualifications

• Must possess a current Arizona RN license.
• BLS certification is required.
• ADN or Diploma nursing degree; BSN preferred.
• Two years behavioral health experience required.
• Prior Intake experience is a plus.

Responsibilities

• Assess patient needs and review history with patient/family.
• Formulate and initiate a plan of care with input from patient, family and health team.
• Coordinate and supervise care activities per physician orders and policies.
• Document assessments and care plans timely in the patient record.
• Assist with admissions and determine patient benefits/eligibility.
