Job summary

Portsmouth Regional Hospital is seeking a Registered Nurse (RN) for Behavioral Health to provide patient-centered care at a 240-bed acute care facility in Portsmouth, NH. The role supports a mission-driven team within a leading healthcare system and involves coordinating care with medical providers and the care team.

The RN will assess patients at admission and during shifts, document care, and collaborate on holistic treatment plans while addressing patient needs and supporting recovery.

Qualifications

• NH RN license required.
• Associate Degree of Nursing required.
• 6 months of RN experience required.

Responsibilities

• Assess patient condition during admission and each shift, report changes in status.
• Document care provided in the patient record promptly and accurately.
• Respond to patient requests promptly and deliver exceptional care.
• Collaborate with a multidisciplinary team to coordinate and customize patient treatment plans.
• Serve as interface between providers and community services for ongoing care.
