As a Clinical Team Manager, you’ll be both a clinical leader and operational strategist. You’ll ensure patients receive coordinated, personalized behavioral health services while fostering a high-performing, mission-aligned care team.
Key Responsibilities:
Lead the delivery of diagnosis-specific behavioral health care in the home
Manage patient care planning, staffing, and coordination across providers and facilities
Oversee timely initiation of care in compliance with all regulations
Approve and update care plans and physician orders
Guide clinical staff through regular communication, performance oversight, and coaching
Support QAPI (Quality Assurance and Performance Improvement) initiatives to enhance outcomes

Education:
Associate’s Degree in Nursing (required)
Bachelor’s in Nursing or related field (preferred)
Experience:
Minimum 2 years as a Registered Nurse in behavioral health or home health
At least 1 year in a leadership or supervisory role
Licensure:
Current and active RN license in the state of practice
Tech Savvy:
Experience with clinical enterprise systems (e.g., HomeCare HomeBase, Workday)
Proficient in Microsoft Office Suite (Word, Excel, Outlook)
