Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Home Health Registered Nurse to join our Wise, VA team. This role focuses on 1:1 patient care in a home setting with a Friday–Sunday schedule, offering stability, growth opportunities, and a supportive work environment.

You will assess patient needs, develop care plans, administer treatments, educate families, and coordinate with an interdisciplinary team to ensure high-quality outcomes in the home health setting.

Qualifications

• Registered Nursing Degree (Associate or Bachelor) with current unrestricted registration and license in the state of practice and residence.
• One year nursing practice in a patient care setting required; home health, geriatrics or related settings preferred.
• Current CPR certification required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient families and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.

Education

• One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred
• Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy
• New nursing graduates may be considered in select markets based on program availability
• Current CPR certification
