Job summary

Behavioral Health Network (BHN) is seeking an Assessment Nurse to join its One Care team in Worcester, MA. The role focuses on conducting health assessments, developing person-centered care plans, and educating individuals and families on health matters.

Ideal candidates hold an MA RN license, with preferred experience in behavioral health and substance use disorders, and bilingual Spanish/English abilities. A strong commitment to patient dignity and culturally competent care is essential.

Qualifications

• Current Registered Nurse license with Massachusetts Board of Registration; must maintain licensure.

Responsibilities

• Conduct initial and annual health assessments.
• Collaborate with individuals served to develop person-centered care plans.
• Maintain professional relationships with funding sources, staff, and stakeholders.
• Educate individuals served and families on health-related matters.
• Promote InstED emergency department diversionary services when appropriate.
• Remain familiar with One Care policies and procedures and updates.

What You Will Do:

• Conduct initial and annual health assessments in a way that promotes individuals served health, safety, dignity, and respect.
• Collaborate with individuals served to develop person-centered care plans.
• Maintain a collaborative and professional relationship with the funding source, staff, and stakeholders to support the care of individuals served.
• Educate individuals served and their families/supports on relevant health-related matters as appropriate.
• Promote the use of InstED emergency department diversionary services with individuals served and their families/supports.
• Maintain familiarity with One Care policies and procedures and adhere to any updates or changes.
• Complete documentation in accordance with program policies and timeframe requirements, and complete any additional program-specific forms and documents as required in a timely manner.
• Attend in-services, meetings, and training courses as assigned and scheduled, and keep mandatory trainings up to date.
• Complete Critical Incident and Mandated Reporting processes as needed.

Who You Are:

• High School Diploma or GED required.
• Current Registered Nurse License by the Massachusetts Board of Registration; must maintain current RN licensure and certifications.
• Demonstrated experience providing services to individuals with behavioral health and substance use disorders strongly preferred.
• Bilingual Spanish and English speaking preferred.
• Valid driver's license and reliable, legally registered vehicle available for all working hours required.
• Commitment to consumer empowerment, growth, human rights, and community integration.
• Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance use, and care management.
• Professional demeanor in communicating with individuals served, colleagues, and other providers.
• Demonstrates cultural and linguistic competence in working with individuals of varied cultural backgrounds and linguistic needs.
• Exemplary and culturally sensitive customer service skills directed toward individuals served.
• Demonstrates willingness and ability to serve challenging and diverse populations.
• Familiarity with Internet, email systems, and electronic health records (EHRs).
• Strong verbal and written communication skills; ability to communicate clearly and effectively with individuals served, providers, and staff.
• Working knowledge of program KPIs and performance metrics and how they impact the role, the program, and the overall mission of the One Care program.

Salary Range: $40-$46/hr
