Job summary

Behavioral Health Network (BHN) is seeking an Assessment Nurse to join our One Care team in Western Massachusetts. This role conducts initial and annual health assessments, develops person-centered care plans, and collaborates with staff and families to support the care of individuals served.

The RN must hold an active MA license, bring strong communication skills, and work effectively with diverse populations.

Qualifications

• High School Diploma or GED required.
• Current Registered Nurse License by the Massachusetts Board of Registration; must maintain licensure.
• Bilingual Spanish and English speaking preferred.
• Valid driver’s license and reliable vehicle for all working hours.
• Knowledge of mental illness and care management.

Responsibilities

• Conduct initial and annual health assessments with respect for dignity.
• Develop person-centered care plans with individuals served.
• Maintain professional relationships with funding source, staff, and stakeholders.
• Educate individuals and families on health-related matters as appropriate.
• Promote InstED emergency department diversionary services with individuals served and their families/supports.
• Maintain familiarity with One Care policies and procedures and adhere to updates.
• Complete documentation per program policies promptly.
• Attend trainings and meetings; keep mandatory trainings up to date.
• Complete Critical Incident and Mandated Reporting processes as needed.
