Chestnut Hill Lodge Health and Rehabilitation Center in Wyndmoor, PA is seeking an RNAC Registered Nurse Assessment Coordinator for a Mon-Fri, on-site role. The position focuses on coordinating admissions assessments, developing individualized care plans with an interdisciplinary team, and ensuring compliant documentation for reimbursement.

Qualifications

• Prior experience as an RNAC or MDS Coordinator required.
• Current RN license in the State of Pennsylvania.
• Strong assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• MDS Coordination: Overseen timely and accurate MDS assessments.
• Care Planning: Collaborate with doctors, therapists and social workers to update care plans.
• Reimbursement Compliance: Ensure documentation supports Medicare/Medicaid and PDPM.
• Quality Monitoring: Track quality measures and drive facility improvement.
• Audits and Surveys: Prepare documentation and assist during regulatory audits.
