Serenity Spring Senior Living at Jasper Rehabilitation & Nursing Center in Jasper, IN is seeking a compassionate Registered Nurse Assessment Coordinator (RNAC) to assess patients, coordinate care plans and ensure regulatory compliance. The RNAC will document MDS and support the care team to improve patient outcomes.

The role requires strong communication, teamwork, and proficiency with EHR systems, with a focus on accuracy and high-quality care for our residents.

Qualifications

• Must hold an active RN license in Pennsylvania.
• Minimum 1 year of experience in long-term care or similar settings.
• Strong knowledge of MDS assessments and Medicare/Medicaid regulations.
• Excellent communication and interpersonal skills.
• Detail-oriented with accuracy and regulatory compliance.
• Proficient with electronic health records (EHR).

Responsibilities

• Conduct comprehensive patient assessments to determine care needs.
• Collaborate with healthcare professionals to develop and update care plans.
• Ensure compliance with Medicare and Medicaid regulations and guidelines.
• Accurately complete and submit MDS assessments.
• Monitor patient progress and adjust care plans as necessary.
• Provide education and support to clinical staff regarding assessment and documentation best practices.
• Participate in interdisciplinary team meetings to discuss patient care and coordination.
• Maintain accurate and up-to-date patient records.
