You will conduct resident assessments, develop care plans, ensure documentation for reimbursement, and coordinate care with the leadership team to meet regulatory requirements.

Qualifications

• Valid PA RN license and CPR certification
• Minimum 1 year long-term-care experience; supervisory experience preferred
• At least 2 years of experience with completing MDS and interdisciplinary care planning
• Strong interpersonal skills to support, advise, coach, and influence others
• Proficiency with MDS software as well as PC and Microsoft Office applications

Responsibilities

• Conduct thorough assessments of residents' medical and care needs and develop care plans.
• Ensure accurate documentation for reimbursement and regulatory compliance.
• Coordinate care delivery and monitor resident progress.
• Support and communicate with the leadership team, residents, and family members.
• Maintain compliance with regulatory requirements and quality standards.
• Foster effective collaboration among interdisciplinary care teams
