Job summary

NHC HealthCare Greenwood is seeking a Registered Nurse to serve as the RN, Assessment Coordinator, coordinating patient assessments and MDS coding guidance within a geriatric care setting.

The ideal candidate holds SC RN licensure, has geriatric experience, and is knowledgeable about the RAI process and SNF Part A Medicare rules. This role offers a stable schedule and opportunities for advancement within a family‑oriented team.

Qualifications

• Registered Nurse licensed in SC, graduate of an accredited nursing program.
• CMAC certification preferred.
• Professional experience in geriatrics nursing.
• Knowledge of the Resident Assessment Instrument and applicable rules.

Responsibilities

• Schedule and manage deadlines with organizational skills.
• Monitor medical records to validate support of MDS coding.
• Collaborate with interdisciplinary team to ensure timely Care Area Triggers and Patient Care Plans.
• Support accuracy of medical record documentation through ongoing in-services to center partners.
