Jethro Health is seeking an experienced Home Health Agency RN to serve patients in the North Shore area, including Lynn, Peabody, Nahant, Revere, Winthrop, East Boston, Chelsea, Salem, Swampscott, Wakefield, Reading, Woburn, Saugus, and Revere.

We are looking for a compassionate and dedicated LPN or RN to join our Home Health team. The ideal candidate will provide quality care in patients' homes, promoting their well-being and independence.

This is a Per-Diem or Part-Time position, with potential to become Full-Time.

Responsibilities

1. Conduct thorough assessments of patients' health status and needs in their home environment.
2. Develop and implement individualized care plans based on assessments and medical history.
3. Educate and support patients and families on health conditions, medications, and self-care.
4. Assist with discharge planning from hospitals or rehab facilities for smooth home transition.
5. Monitor patient progress and adjust care plans as needed.
6. Collaborate with interdisciplinary teams for comprehensive care.
7. Document all patient interactions accurately, adhering to ICD-10 standards.
8. Offer health coaching to empower patients in managing chronic conditions.
9. Provide specialized services such as esthetic laser treatment when applicable.
10. Support family planning initiatives and educate patients on related health topics.

Skills

Pay: RN - Oasis Start of Care: $100 per visit; RN - Recertification: $60 per visit; RN - Oasis Resumption of Care: $80 per visit; RN - Skilled Nursing visit: $45 per visit; LPN - Skilled Nursing visit: $40 per visit. Pay range: $37.73 - $45.00/hour.
