Job summary

Heart to Heart Hospice is seeking a Registered Nurse On Call to deliver compassionate home-based care for patients with life-limiting illnesses. You will assess, treat, and support patients and families during challenging times, including crisis moments and end-of-life needs.

The role emphasizes plan development with physicians and the IDG, thorough documentation, excellent communication, and adherence to laws and policies. A valid RN license and driving ability are required.

Qualifications

• Graduate of an accredited nursing program (Diploma, Associate, or Baccalaureate).
• Current State RN license and valid driver's license.
• Two years of RN experience preferred.

Responsibilities

• Provide home visits for assessment, treatment, and support.
• Develop and revise nursing care plans with physician and IDG.
• Demonstrate excellent customer care with staff and patients.
• Prepare progress notes showing progress toward goals.
• Inform physician of changes in patient needs and response to care.
• Ensure timely documentation and compliance with laws and policies.
• Perform other duties as required.

Registered Nurse On Call

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as requiredEnsure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
