Heart to Heart Hospice is seeking a Registered Nurse On Call - Part Time in Mishawaka, IN. The role includes weekend shifts (9am-9pm) and home visits for patient assessment, treatment, and support.

You will plan nursing care with physicians and the care team, while ensuring timely documentation and compliance. The position requires an RN license, a nursing degree (Diploma/Associate/Baccalaureate), a valid driver’s license, and ideally 2 years of RN experience in a healthcare setting.

Qualifications

• Nursing degree from accredited diploma, associate, or baccalaureate program.
• Current State RN license.
• Valid driver's license with reliable transportation and ongoing auto liability insurance.
• Two years of RN experience preferred in healthcare/clinical setting.

Responsabilités

• Provide home visits for patient assessment, treatment, and support (may include pronouncement of death, crisis management, and admission).
• Develop/update nursing care plans with goals in collaboration with physician and IDG.
• Utilize strong customer service skills with internal and external customers; maintain phone etiquette.
• Prepare clinical/progress notes showing progress toward goals.
• Inform physician or staff of changes in patient needs and responses to care; document outcomes.
• Submit all documentation promptly.
• Perform other duties as required; ensure compliance with laws, Medicare regulations, and policies.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
