Job summary

Heart to Heart Hospice seeks an On-Call Registered Nurse to deliver compassionate home-based care. You will perform patient assessments, plan and adjust nursing care with physicians, and provide crisis support as needed. Strong documentation, communication, and adherence to regulatory standards are essential.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and auto liability insurance.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Provide home visits for patient assessment, treatment and support, including pronouncement of death or crisis management.
• Develop or revise nursing care plans in collaboration with physician and IDG.
• Demonstrate excellent customer care with internal and external customers, including phone etiquette.
• Prepare clinical/progress notes showing progress toward goals.
• Inform physician and staff of changes in patient needs and outcomes; evaluate responses to care.
• Timely submission of all documentation.
• Perform other duties as required; ensure compliance with laws, regulations, and policies.

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
