Job summary

Home Health Care, Inc. in the Twin Cities Metro Area is seeking an experienced weekend RN Admissions Nurse to focus on Start of Care admissions without ongoing caseload.

You’ll assess new patients, perform OASIS, and establish initial care plans, earning $200 per Start of Care visit plus mileage reimbursement. This weekend-only role lets you keep weekday commitments, while applying your expertise in home health admissions, clinical decision-making, and comprehensive patient education to ensure

Qualifications

• Active Minnesota RN license required.
• Current CPR certification.
• Previous home health nursing experience preferred.
• Strong OASIS knowledge.
• Start of Care admission experience preferred.

Responsibilities

• Complete weekend Start of Care admissions for new home health patients.
• Perform comprehensive skilled nursing assessments.
• Complete OASIS and admission documentation accurately and on time.
• Review medications, diagnoses, symptoms, safety concerns, and caregiver needs.
• Develop and initiate individualized plans of care.
• Provide patient and caregiver education.
• Coordinate care with physicians, nurses, therapists, and other interdisciplinary team members.
• Communicate significant clinical findings and changes appropriately.

What You'll Do

Who We're Looking For

This is not intended to be an entry-level home health opportunity. We're looking for an RN who can confidently manage the complexity of an admission in the field.

Required Qualifications

• Active Minnesota Registered Nurse license.
• Current CPR certification.
• Previous home health nursing experience.
• Strong OASIS knowledge.
• Start of Care or admission experience strongly preferred.
• Strong assessment and clinical decision-making abilities.
• Excellent documentation and communication skills.
• Valid driver's license, active insurance, and reliable transportation.
