Job summary

Home Health Care, Inc. is seeking a Weekend Home Health RN to focus on Start of Care admissions in patients’ homes. You’ll earn $200 per Start of Care visit, with mileage reimbursement, and work weekends only to supplement existing nursing income.

This role emphasizes independent practice, comprehensive assessments, and care coordination with the interdisciplinary team to set up a successful episode of care.

Qualifications

• Active Minnesota RN license must be current and in good standing.
• Current CPR certification is required.
• Previous home health experience is required.
• Strong knowledge of OASIS and Start of Care admissions.
• Excellent clinical assessment and documentation skills.
• Ability to work independently in patients' homes.
• Valid driver's license and auto insurance are required.

Responsibilities

• Complete weekend Start of Care admissions in patients' homes.
• Perform comprehensive nursing assessments and complete OASIS documentation.
• Develop and initiate individualized plans of care.
• Identify clinical, medication, safety, and disease-management needs.
• Educate patients and caregivers on medications, diagnoses, symptoms, and home care.
• Coordinate services with nurses, therapists, physicians, and other team members.
• Complete accurate and timely electronic documentation.

What We're Looking For

• Active Minnesota Registered Nurse (RN) license.
• Current CPR certification.
• Previous home health experience required.
• Strong working knowledge of OASIS and Start of Care admissions.
