Responsibilities

  • Admissions & clinical assessment: coordinate all aspects of resident admissions and review hospital records and orders.
  • Care coordination: communicate admission details to departments and ensure timely orders and care plans.
  • Clinical nursing support: assess conditions, administer medications and IV therapy as needed, respond to emergencies.
  • Documentation & compliance: maintain accurate admission records and ensure HIPAA and regulatory compliance.
  • Resident & family relations: educate residents and families about care plans and address questions throughout the admission process.

In this key role, you will coordinate new resident admissions, perform clinical assessments, oversee care transitions, and help ensure exceptional outcomes for our residents.If you thrive in a fast-paced healthcare environment, have strong clinical assessment skills, and enjoy collaborating with interdisciplinary teams, we'd love to hear from you.## Position SummaryThe **RN Admissions Nurse** is responsible for coordinating and facilitating the admission process for new residents while ensuring accurate clinical assessments, documentation, regulatory compliance, and seamless transitions of care. This position works closely with physicians, hospital discharge planners, therapy teams, nursing staff, residents, and families to deliver a positive admission experience and maintain high-quality patient care.## ## Key Responsibilities### ### Admissions & Clinical Assessment* Coordinate and manage all aspects of resident admissions* Complete comprehensive nursing assessments upon admission* Review hospital records, physician orders, medications, and treatment plans* Ensure accurate and timely admission documentation* Verify insurance, authorization, and required clinical information* Facilitate smooth transitions from hospitals and other healthcare settings### ### Care Coordination* Collaborate with physicians, nursing staff, therapy, pharmacy, and interdisciplinary teams* Communicate admission details and care needs to appropriate departments* Ensure timely implementation of physician orders and care plans* Coordinate laboratory services, diagnostic testing, and follow-up care* Act as a liaison between residents, families, and the care team### ### Clinical Nursing Support* Assess resident conditions and identify changes in status* Administer medications, treatments, and IV therapy as needed* Respond appropriately to medical emergencies* Support residents in achieving the highest level of independence and well-being* Provide clinical guidance and support to nursing staff during the shift### ### Documentation & Compliance* Maintain accurate and complete admission records* Ensure compliance with federal, state, and facility regulations* Monitor documentation accuracy and regulatory requirements* Participate in quality improvement initiatives and audits* Maintain confidentiality and uphold HIPAA standards### ### Resident & Family Relations* Educate residents and families regarding care plans and facility services* Address questions and concerns throughout the admission process* Promote a welcoming and compassionate admission experience* Maintain professional and effective communication with residents and families## ## Qualifications### Required* Active **Registered Nurse (RN)** license in California* Graduate of an accredited nursing program* Minimum **2 years of RN experience*** Strong clinical assessment and care coordination skills* Excellent communication, organization, and documentation abilities### ### Preferred* Experience in **Skilled Nursing Facilities (SNF)**, Post-Acute Care, or Long-Term Care* Admissions nursing experience strongly preferred* Experience with **PointClickCare (PCC)** or other Electronic Health Record (EHR) systems* BSN preferred## ## Why Join Arden Park Post Acute?* Competitive pay: **$48.00–$50.00 per hour DOE*** Full-time, stable schedule* Supportive leadership team* Collaborative clinical environment* Career growth and advancement opportunities* Opportunity to make a meaningful impact on resident care and outcomes## ## About Arden Park Post AcuteAt Arden Park Post Acute, we are committed to delivering compassionate, high-quality skilled nursing and rehabilitation services.
