Job summary

Community Healthcare of Texas is seeking a Field RN to join our hospice team in Fort Worth, Texas. This PRN role requires travel to patient locations, assessment, and coordination of care with families and the Case Manager.

You will provide compassionate nursing care, educate patients and caregivers, and document assessments and plans of care. Qualified candidates are licensed RNs in Texas with strong computer skills and reliable transportation; hospice experience is preferred.

Qualifications

• Licensed RN in Texas.
• 2+ years of nursing experience required; hospice experience preferred.
• Strong computer skills and proficiency with electronic health records.

Responsibilities

• Meet with patients and families to explain hospice services and plan of care.
• Coordinate admission of patients and ongoing care with the Case Manager.
• Assess patient condition and revise care plans as needed.
• Provide emotional support to patients and families.
• Complete daily paperwork and charting per policy.
• Evaluate hospice appropriateness and document findings.

Co mmunity Healthcare of Texas is currently recruiting for a Field RN.

POSITION SUMMARY

Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.

ESSENTIAL FUNCTIONS

• Assess patient's physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.
• Provide emotional support to patients and families while instructing them in activities of daily living.
• Complete daily paperwork and charting, thoroughly and accurately, according to Policy.
• Evaluate the patient for hospice appropriateness and complete admission visit and documentation as needed; communicate with the Case Manager on significant findings.

POSITION QUALIFICATIONS

• Registered Nurse in the State of Texas
• Hold a valid driver's license and have a dependable means of transportation.
• 2 years of nursing experience required
• 2+ years of experience in hospice strongly preferred
• Proficient skill using a computer is required; Microsoft Office Suite experience preferred.
