White Plains Hospital Medical Center seeks an experienced RN to join the Acute Care Transition Team (ACTT). The ACTT RN bridges outpatient and inpatient care, coordinating discharge planning and follow-up across hospital and home environments.

Responsibilities include post-discharge outreach within 48 hours, medication management, social determinants assessment, and collaboration with clinicians, social workers, and care navigators.

Qualifications

• New York State RN license is required.
• BSN required for RNs hired after 1/1/18.
• All RNs hired before 1/1/18 must have BSN/MSN by 12/31/21.

Responsabilités

• Bridge the gap between outpatient and hospital-to-home transitions.
• Coordinate discharge follow-up and assess post-acute needs.
• Manage medications and care plans during follow-up discussions.
• Assess social determinants of health impacting recovery.
• Communicate with patients, families, and care teams to ensure continuity of care.
