Job summary

Kaiser Permanente in Arlington, VA is seeking a Nurse Patient Care Coordinator to oversee care coordination for the acute inpatient population. The PCC collaborates with MAPMG Hospital Based Service Physicians, patient/family, nursing, utilization review and other members of the healthcare team to assure safe and cost-effective discharge and prevent readmission.

Qualifications

Minimum of one year in an acute medical/surgical/ED or critical care nursing area.
Associate Nursing degree required.
RN license in VA within 6 months of hire; Maryland/DC licenses as applicable.
Basic Life Support certification.

Responsibilities

Completes an initial face-to-face assessment for every admitted member within 24 hours of admission.
Document assessment outcomes in the medical record and communicate discharge plans to care teams.
Participate in daily Care Without Delay rounds and monitor progression toward discharge.
Coordinate with social workers and post-acute services for safe discharge and readmission prevention.
Identify and address delays in care, offering solutions to prevent time lags in discharge.
