Job summary

St. Luke’s University Health Network seeks a Registered Nurse for its Assertive Community Treatment (ACT) Team. You will deliver nursing care in clients’ homes and communities, supporting symptom management and coordination with the ACT multidisciplinary team.

Responsibilities include medication management, treatment planning, crisis support, and timely MAR documentation, with rotational on-call duties to meet program needs.

Qualifications

• Experience in psychiatric nursing specialty area.
• Evidence of Basic Life Support (BLS) certification.
• CPR certification required.
• Must possess and maintain a valid driver's license.

Responsibilities

• Meet ACT participants in home and community settings to improve quality of life and community tenure.
• Provide medication service and observation of ingestion as directed by clinicians.
• Coordinate treatment goals and provide individual counseling for symptom management.
• Provide on-call crisis services on a rotating schedule to cover program needs.
• Document all services timely in the electronic health record.
