Diamond Care Group is seeking a Home Health Registered Nurse in Port Charlotte, FL to join our expanding team. This role entails patient assessments, care plan development, and collaboration with the care team to ensure quality outcomes.

Ideal candidates hold an active FL RN license with at least 1 year of home health experience, strong communication, and the ability to travel between patient homes. We offer full-time positions with benefits and support from a Clinical Manager.

Qualifications

• Current Registered Nurse license in Florida.

Responsibilities

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated.
• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.

Qualifications
