Job summary

Diamond Care Group is expanding and seeks dedicated Home Health RNs for FT and PRN roles. You will perform comprehensive patient assessments, develop care plans, and coordinate with a multidisciplinary team in patients' homes.

The ideal candidate holds an FL RN license, at least 1 year of home health experience, and familiarity with Kinnser. Excellent communication and organization are essential; travel between patient homes is required.

Qualifications

• Current Registered Nurse (RN) license in Florida.
• At least 1 year of home health nursing experience.
• Excellent knowledge of nursing care methods and procedures.
• Strong communication and interpersonal skills; team player.

Responsibilities

• Perform comprehensive patient assessments and develop care plans with the care team.
• Review medications and vital signs and complete evaluation tasks.
• Coordinate care by making referrals or documenting rationale when referrals aren’t indicated.
• Communicate with the clinical manager and interdisciplinary team about patient status and updates.
• Educate patients and caregivers to promote independence and health outcomes; train in self-care techniques.
• Coordinate with physicians and therapists in the patient’s care plan.
• Supervise LPNs to ensure high standards of care.
• Perform additional duties to support patients and team operations.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated.
• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.

Qualifications
