Job summary

Diamond Care Group seeks an experienced Home Health Registered Nurse to join our Florida-based team. This role focuses on performing patient assessments, developing individualized care plans, and coordinating with physicians and therapists to ensure high-quality home care.

The RN will educate patients and families, supervise LPNs, and maintain thorough documentation in our EHR system (Kinnser). A Level 2 background check is required prior to employment.

Qualifications

• Current Registered Nurse (RN) license in Florida.
• Minimum 1 year experience as a home health nurse.
• Proficient in nursing care methods and procedures.
• Knowledge of health and safety guidelines and procedures.
• Team player with strong communication and interpersonal skills.
• Excellent organizational and multitasking abilities.
• Comfortable with travel between patient homes.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans with the care team.
• Review medications and vital signs as part of evaluations.
• Coordinate care by referring or documenting clinical rationale when referrals are not indicated.
• Communicate with the clinical manager and interdisciplinary team about patient status and updates.

• Coordinate with Physicians, Physical Therapists, Occupational Therapists and others in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties to support patients and team operations.

• Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.
• Complete evaluation tasks, including reviewing medication and vital signs.

• Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
• Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
