Job summary

Palms-Home-Care is seeking Home Health RNs for full-time and PRN roles to join a compassionate team delivering patient-centered care in the field. You will perform assessments, develop care plans, and collaborate with physicians, therapists, and the care team to promote independence and better health outcomes.

The role requires an active Florida RN license, 1+ year home health experience, and strong communication, organization, and problem-solving abilities.

Qualifications

• Current Registered Nurse (RN) license in the state of Florida.
• Must have at least 1 year experience as a home health nurse.
• Excellent knowledge of nursing care methods and procedures.
• In-depth knowledge of health and safety guidelines and procedures.
• A team player with excellent communication and interpersonal skills.
• Outstanding organizational and multi-tasking skills.
• Patient with excellent problem-solving skills.
• Familiar with EHR such as Kinnser.
• Ability to work independently, manage a daily schedule, and travel between patient homes.

Responsibilities

• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
• Supervise and support LPNs to ensure high standards of care.
• Perform additional duties as needed to support patients and team operations.
