Description

Summit Home Care and Hospice is searching for an experienced and motivated Registered Nurse (RN) to join our Home Health team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.

Position Details
  • Shift: Dayshift, Monday – Friday (40 Hours/Week) 8am to 5pm est
  • Annual Salary: $70,000 - $80,000 (Overtime Eligible)
  • Location/Territory: Cincinnati Ohio 45238
  • Expected Travel: 30 Mile Radius (Mileage Reimbursement, $0.48/Mile)
Responsibilities
  • The RN plans, organizes, and directs home care services, emphasizing community health education/experience.
  • Builds from community resources to plan and direct services to meet individual and family needs at home and in the community.
Patient Care
  • Complete initial assessments to determine home care needs.
  • Provide physical assessments and histories of current and previous illnesses.
  • Re-evaluate patient needs regularly.
  • Develop and revise care plans based on patient status and needs.
  • Use assessment data to determine nursing diagnoses.
  • Develop care plans with goals, incorporating therapeutic, preventive, and rehabilitative actions, involving patients and families.
  • Administer medications and treatments as prescribed.
  • Provide direct patient care as per State Nurse Practice Act.
  • Counsel patients and families, and provide health care instructions.
  • Identify discharge planning needs and implement prior to patient discharge.
  • Act as Case Manager when assigned, coordinating patient care.
Communication
  • Prepare clinical notes and update physicians as needed, at least every 60 days.
  • Communicate patient needs and changes to physicians, and obtain orders as required.
  • Coordinate care with community health providers.
Additional Duties
  • Participate in on-call duties as per policy.
  • Ensure availability of equipment and services.
  • Supervise and evaluate home health aide care biweekly.
Qualifications
  • Graduate of an accredited nursing program, with an associate or bachelor’s degree.
  • Valid Ohio nursing license.
  • Valid driver’s license, insured vehicle.
  • CPR certification, TB test.
  • Preferred: home health or acute care experience.
  • Management experience is a plus.
  • Strong communication, problem-solving, and nursing skills.
  • Physical ability to walk, stand, lift, and perform nursing duties.
