Job summary

JourneyCare Home Health is seeking a skilled Registered Nurse to join our field-based team delivering nursing visits in patients' homes. The RN will perform assessment, care planning, and coordination with the interdisciplinary team to support recovery.

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• 1+ year clinical nursing experience preferred in home health, ICU, rehab, or medical-surgical settings.
• CPR certification required.

Responsibilities

• Provide visits as assigned by the Clinical Services Manager, including assessment, eligibility, admissions, planning, implementation and evaluation phases of the nursing process.
• Assess data from patient and family factors and coordinate interventions with the interdisciplinary team.
• Initiate communication with attending physician and other agencies to coordinate care.
• Update plan of care based on patient needs and visits.
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the physician.
• Provide end-of-life support as applicable to licensure.
• Maintain accurate patient records and care plans in a timely manner.

JourneyCare Home Health is seeking a skilled and compassionate Registered Nurse to join our team. This field-based position is responsible for case management, providing skilled nursing services, and supporting patients on their road to recovery in the comfort of their homes.

Monday-Friday, 8am-5pm

What You'll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Graduate from an accredited registered nursing program and licensed in the state
• At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
• Home health experience preferred
• Possess and maintain valid CPR certification.
• Must have reliable transportation, current driver's license and appropriate automobile insurance
