Addus Homecare in Dickson, TN is seeking a full-time Registered Nurse (RN) to provide one-on-one skilled nursing visits in patients’ homes across Dickson County and surrounding areas. The role involves weekday visits with a rotating on-call schedule, including evenings and weekends, with hours varying by patient needs and travel.

As an RN you will perform assessments, plan and implement care, coordinate with physicians and the interdisciplinary team, maintain accurate records, and support

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• At least one year clinical nursing experience; home health experience preferred.
• CPR certification required and reliable transportation.

Responsibilities

• Provide visits as assigned, including assessment, eligibility, admissions processing, planning and evaluation.
• Coordinate care with the interdisciplinary team and attending physician.
• Maintain accurate patient records and update care plans timely.
• Perform nursing procedures as directed and support family as applicable.
