Job summary

Rever Grand, Inc. is seeking a Risk Support Specialist to join the administrative team serving individuals with I/DD.

You will collaborate with individuals or representatives to create person-centered Risk Support Plans and Medication Protocols for Direct Support Professionals, ensuring deadlines and compliance with state guidelines. The role emphasizes accurate data collection, confidential handling of information, and effective communication via phone, email, and text with diverse contacts.

Qualifications

• High school diploma or equivalent.
• 3+ years of experience in customer service and/or data entry.
• Experience with individuals with I/DD is a plus.
• Ability to maintain confidentiality.
• Ability to learn multiple software systems.
• Strong written and verbal communication.

Responsibilities

• Collect and interpret information to develop the Risk Support Plan for individuals.
• Develop and write Risk Support Plans and Medication Support Protocols.
• Review ISP and other documents to gather needed information.
• Communicate with individuals/representatives to determine preferred support.
• Conduct research to ensure thorough risk and medication plans.
• Ensure plans meet state guidelines and deadlines.
• Perform annual reviews and updates to plans as needed.
• Maintain strict confidentiality and accurate data entry.
