Job summary

Responsibilities

• Review medical records to identify coding based on CMS HCC categories.
• Prepare medical charts and track patient information via Excel spreadsheets.
• Document claim/encounter information and support coding recommendations.
• Provide coding education and training on documentation quality and coding guidelines.
• Coordinate with Primary Care Providers for MRA coding, HEDIS and STARS.
• Monitor coding changes to ensure up-to-date information.
• Work HCC suspect reports and submit encounters timely.
• Research code questions for multiple provider offices as directed.
• Update Director on status weekly; notify when wellness visits are not scheduled.
• Travel to offices for on-site chart reviews; assist with billing claims.
