Job summary

The University of Kansas Health System is seeking an Ambulatory Clinic RN Care Coordinator in Rheumatology at Bell Hospital, South KC Medical Pavilion. Responsibilities

• Manage patient flow, triage, and direct patient care.
• Coordinate communication among front desk, nursing staff, physicians, and clinic manager.
• Develop patient outcomes and goals with providers to optimize care.
• Provide nursing support and coordination to improve patient outcomes.
• Educate patients and families about plans of care and self-management.
• Document assessments and progress in EMR.
