Job summary

Advocate Health Lake Geneva location is seeking a full-time Pharmacist to oversee pharmaceutical care and design patient-specific regimens. You will educate patients and caregivers on therapy, monitor safety, and ensure formulary adherence while collaborating with the healthcare team.

Your role includes precepting students, guiding staff, and maintaining compliance with state and federal regulations in a dynamic retail pharmacy setting.

Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity
• Immunization and non-vaccine injectable certification may be required
• Credentialing and privileging required based on area of practice
• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school or NABP FPGEC certificate

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens
• Provides patient and caregiver counseling on medication therapy, adverse effects, compliance and administration
• Implements evidence-based pharmacotherapeutic regimens and documents in required formats
• Interprets medication orders and prescriptions accurately
• Participates in medical emergency management when applicable
• Supports medication safety and quality improvement activities
• Collaborates to manage formulary adherence and medication-use process
• Assists in acquisition of non-formulary, compassionate use, and investigational meds
• Prepares, dispenses, and stores medications per regulations
• Serves as a preceptor for students and residents
• Directs and checks the pharmacy support team's work for safety and efficiency
• Demonstrates age-appropriate patient care knowledge
