Job summary

Little Village Nursing and Rehabilitation Center is seeking a Restorative Nursing Supervisor to oversee day-to-day C.N.A. compliance with restorative nursing functions and programming.

Qualifications

• Current and unrestricted RN/LPN Illinois license, preferably rehab certified.
• Must read, write, speak, and understand English.
• Effective communication with care plan team; supervise CNAs on restorative programs.
• Ability to make independent decisions when warranted.
• Ability to deal tactfully with residents, families, staff, and agencies.
• Knowledge of nursing and medical practices, laws, and guidelines for long-term care.
• Leadership and supervisory abilities; able to work harmoniously with staff.
• Physically capable of hearing, seeing, speaking, and mobility requirements (stairs, walking, 150 lbs+).
• Able to participate in evaluation of residents during emergencies.
• Experience with Medicare/State reimbursed services and documentation requirements.

Responsibilities

• Assess, develop, and manage restorative nursing care plans with the IDT.
• Determine changes to care plans based on ongoing assessments to promote safety.
• Attend weekly Medicare Meetings and Care Plan Conferences.
• Coordinate programming to address residents’ restorative needs (e.g., fall prevention, splints).
• Direct CNAs to ensure compliance with restorative program elements and timely documentation.
• Ensure all restorative program documentation is completed accurately and timely.
• Assess residents for restorative needs with the IDT.
• Monitor CNAs in delivering restorative care.
• Evaluate new admits within 24 hours for rehab needs.
• Provide training, in-servicing, and disciplinary actions as warranted.
• Develop staff in-services on restorative and rehab programming and documentation.
• Maintain emergency and disaster policy adherence.
• Maintain charting per policies; ensure a safe environment.
• Participate in CNAs performance evaluations; address non-compliance.
• Ensure HIPAA compliance and safeguarding of computer systems.
