Job summary

Veterans Health Administration in Pittsburgh, PA, is seeking a Registered Nurse- Restorative Coordinator to lead restorative care within the Community Living Center at the Heinz Campus. You will oversee licensed practical nurses and nursing assistants and work with the interdisciplinary team to promote safe transitions and maximize veterans’ independence.

The role includes developing educational programs, monitoring patient progress, and implementing restorative care strategies.

Qualifications

Eligibility requires a full and unrestricted RN license in any US state or territory.

Responsibilities

Direct nursing education, orientation, competencies and quality improvement initiatives.
Partner with residents, families and interdisciplinary team to promote safe transitions across the health care continuum.
Evaluate residents in the Restorative Care Program on admission and monthly to determine potential for the program.
Screen Veterans and conduct initial assessments prior to admission to Restorative Care Program.
Outline and implement measurable functional goals to maximize independence and functioning.
Collaborate with OT/PT in recruiting and admitting patients and developing independence goals.
Monitor progress monthly and adjust therapy, goals and activities as needed.
Serve as role model and mentor to staff; provide educational programs and in-services for restorative care competencies.
