Sierra Vista Post-Acute in Modesto, CA is seeking a Restorative Nursing Assistant to support the maintenance program under therapists' orders and document ADLs, I&Os, vitals, and meal percentages. You will assist with mobility, ROM, and transfers while ensuring resident dignity and rights in a collaborative care setting.

The role requires a CA-licensed CNA, RNA certification, and active BLS/CPR. This is a full-time, on-site position offering a supportive team, orientation, and comprehensive

Qualifications

Must be a licensed Certified Nursing Assistant in California.
Current RNA Certification.
Active BLS/CPR

Responsibilities

Perform restorative/rehabilitation care (ambulation, ROM, etc.) as per individual care plans.
Document resident participation and response to services per state regulations and facility policy.
Recommend modifications to residents' restorative care.
Collaborate with Nursing staff and licensed rehab staff to ensure services meet resident needs.
Assist with lifting, turning, moving, positioning, and transporting residents.
Respect residents' rights and treat all residents with kindness, dignity, and respect.
