Job summary

Lincoln Village Healthcare Center in Lincoln, IL, seeks a Restorative Nurse to plan, implement, and evaluate restorative programs improving residents’ function and independence. You will work with nursing, therapy, and other teams to coordinate care and promote quality of life.

The role requires LPN or RN licensure in Illinois, with prior long-term care/restorative experience preferred, plus strong communication and organizational skills to lead staff and ensure compliant care.

Qualifications

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) in Illinois, in good standing.
• Experience in long-term care or restorative nursing preferred.
• Strong communication, leadership, and organizational skills.

Responsibilities

• Develop, coordinate, and implement individualized restorative nursing programs.
• Monitor residents' progress and modify programs for optimal outcomes.
• Collaborate with therapy staff to transition residents from therapy to restorative care.
• Educate residents and families about restorative goals and plans.
• Document interventions and resident progress in the medical record.
