Job summary

Care Plus Management is seeking a Restorative Nurse at Lincoln Village Healthcare Center. The role focuses on planning, implementing, and evaluating restorative programs to help residents maintain function and independence.

You will collaborate with nursing, therapy, and other departments to promote quality of life. Responsibilities include program development, hands-on restorative care, staff training, and thorough documentation to ensure regulatory compliance.

Qualifications

• LPN or RN license in Illinois is required or in good standing.
• Experience in long-term care/restorative nursing preferred.
• Strong communication, leadership and organizational skills.

Responsibilities

• Develop, coordinate, and implement individualized restorative nursing programs as ordered by the physician and care plan.
• Provide hands-on restorative care and educate residents/families on goals.
• Train and supervise nursing assistants in restorative care.
• Document interventions and resident progress in the medical record and ensure regulatory compliance.
