Job summary

Alden Estates of Jefferson is seeking a dedicated Restorative Nurse to lead our restorative nursing program and promote a healing, functional focus across the facility.

You will plan, implement, and supervise restorative care services, document on EMR, and collaborate with nurses and CNAs to optimize ADL performance and fall prevention.

Qualifications

• Must possess a current, unencumbered license to practice as a Registered Nurse in Wisconsin.
• Preferred restorative certification from an accredited school or recognized agency.

Responsibilities

• Plan, develop, organize and direct restorative care services and activities.
• Set up and maintain documentation flow sheets and EMR for daily restorative nursing measures.
• Ensure restorative nursing is reflected in care plans across units.
• Perform or oversee functional assessments on admission and quarterly, guiding nurses on restorative plans.
• Oversee ADL performance and CNA documentation; manage continence and related programs.
• Participate in or oversee fall management and restraint reduction initiatives.

Restorative Nurse

Alden Estates of Jefferson, a renowned post-acute care facility, is seeking a dedicated and compassionate Restorative Nurse to join our team! As a Restorative Nurse, you will play a vital role in developing, implementing, and supervising our restorative nursing program, promoting a restorative nursing philosophy among all facility departments, and ensuring participants achieve their highest level of function.

About the Role:

As a Restorative Nurse, you will be responsible for:

• Planning, developing, organizing, implementing, evaluating, and directing restorative care services, programs, and activities in accordance with current rules, regulations, and guidelines


• Setting up documentation flow sheets and electronic medical record systems for daily documentation of restorative nursing measures


• Overseeing all restorative nursing measures are reflected on the care plan


• Performing or overseeing functional assessments on admission and quarterly as indicated, consulting with nurses in making recommendations for restorative plan of care, and working with nurses in developing restorative plan of care


• Overseeing ADL performance on units, overseeing C.N.A.'s documentation on ADL's, and overseeing incontinent management programs


• Actively participating or overseeing physical restraint reduction programs and fall management programs
