Career Strategies is seeking a dedicated Restorative Nurse in Chicago's South Loop to lead restorative nursing plans that promote independence and mobility. The role focuses on assessment, planning, and collaboration with the care team to enhance residents' quality of life.

You will document progress, educate residents and families, and stay current with best practices in geriatric restorative care. This full-time position is in a long-term care setting.

Qualifications

• Active RN or LPN license in the state of practice.
• Prior experience in long-term care or geriatric nursing preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently as well as collaboratively within a team.

Responsibilities

• Partner with the interdisciplinary team to assess residents and develop comprehensive restorative care plans.
• Implement restorative interventions that support physical, mental, and psychosocial well-being.
• Provide hands-on support and guidance to nursing staff on safe transfer, ambulation, and ADL techniques.
• Monitor resident progress and adjust care plans to ensure optimal outcomes.
• Document interventions, progress, and changes in condition.
• Assess mobility, range of motion, and functional ability; share findings with the care team.
• Coordinate with rehabilitation therapists to align activities with therapy goals.
• Educate residents and families on restorative programs and strategies for independence.
• Maintain a safe, supportive environment that fosters restorative care practices.
