Job summary

PBMR in Chicago, IL seeks a Case Manager to support a caseload of up to fifteen participants, helping them become productive, active community members through employment support, life skills training, crisis intervention, advocacy, and referrals.

Qualifications include a Bachelor's degree in Human Services or equivalent, at least 3 years working with at‑risk youth, strong interpersonal and written communication skills, plus proficiency with MS Office.

Qualifications

• Bachelor's degree in Human Services or equivalent experience.
• At least 3 years of experience supporting and working with at-risk youth.
• Experience documenting case notes and coordinating services.

Responsibilities

• Identify and engage at-risk individuals within the target community.
• Interviews and assesses participant needs for additional services.
• Coordinate between internal and external services and resources.
• Maintain participant records with case notes and events daily in online systems.
• Lead or support job-readiness workshops (computer skills, resume, interviewing).
• Communicate progress in weekly meetings and evaluate program impact.
• Prepare participants' exit plans and post-discharge resources.
• Develop partnerships with community resources and hold partners accountable.
• Operate with high team engagement and adhere to reporting requirements.
