The Restorative Nurse (RN or LPN) Will:

  1. Direct, supervise, and coordinate the Rehabilitation/Restorative Care Program.
  2. Complete required documentation upon resident admission, discharge, or transfer, following facility policies.
  3. Assist in evaluating residents for rehabilitative and restorative services.

Qualifications for Restorative / Fall Nurse (RN or LPN):

  • RN or LPN license.
  • 2-3 years of experience in a SNF setting as a Restorative Nurse.
  • Current restorative/rehabilitation certification.
  • Experience with falls management preferred.
  • Knowledge of the MDS process.
