Job summary

Extended Care LLC in Crystal Lawns, IL is seeking a dedicated Restorative Aide to coordinate, implement and provide restorative nursing care under Nursing and Rehabilitation. The role focuses on maximizing residents' physical function and emotional well-being through tailored programs and collaborative care.

Responsibilities include overseeing restorative plans, documenting progress, and training staff while communicating with care teams and families to support smooth transitions and ongoing

Qualifications

• CNA certification required by state.
• Ability to work every other weekend.
• Compliant with Health Care Worker Background Check requirements.
• Ability to move intermittently throughout the day.
• Ability to cope with mental and emotional stress in a healthcare setting.
• Strong integrity, flexibility and ability to work with residents, staff and support services.
• Ability to work with ill, elderly or distressed residents.
• Physical ability to push, pull, move and lift at least 25 pounds to 5 feet and carry 50 feet.

Responsibilities

• Oversee, direct and participate in restorative programs tailored to residents' needs.
• Communicate with Director of Nursing and Rehabilitation Nurse on goals and care plans.
• Facilitate transitions from skilled rehab to restorative and preventative programs.
• Attend rehab rounds and provide status updates on resident progress.
• Maintain restorative nursing progress and documentation.
• Visit residents daily and ensure productive restorative programs.
• Train primary caregivers in restorative techniques and provide direct care.
• Monitor ROM and Turn & Positioning orders to ensure compliance.
• Assist caregivers with splints, braces and self-help devices.
• Direct and participate in all restorative care programs.
• Assess new admissions to identify restorative needs and relay to care team.
• Communicate resident progress to families in care planning meetings.
• Maintain proficiency with wheelchairs, walkers, canes, lifts, scales and other equipment.
