Job summary

Elevate Care in Niles, IL seeks a compassionate Restorative Aide (CNA required) to support residents' functional independence through restorative care plans. You will assist with exercises, mobility activities, and daily living tasks, track progress, and collaborate with nursing and therapy teams to ensure coordinated care.

Qualifications

Responsibilities

• Assist with Restorative Care Plans and implement individualized plans.

• Collaborate with Nursing & Therapy Teams for coordinated care.
• Maintain accurate, up-to-date care records.

Your Responsibilities:

• Assist with Restorative Care Plans - Work with residents to implement individualized care plans focused on rehabilitation and functional independence.
• Promote Mobility & Function - Help residents with exercises, mobility, and activities of daily living to improve strength and range of motion.
• Track Resident Progress - Monitor and document residents’ improvement and progress in their restorative goals.
• Provide Therapeutic Activities - Lead or assist in therapeutic exercises and activities to maintain or improve residents’ abilities.
• Collaborate with Nursing & Therapy Teams - Ensure consistent, coordinated care for optimal results.
• Maintain Accurate Records - Keep clear and up-to-date documentation of care provided and progress made.

Qualifications:

• Active Certified Nursing Assistant (CNA) Certification
• Previous Experience in Restorative Nursing or Long-Term Care (Preferred).
• Strong Communication & Interpersonal Skills.
