Job summary

Elevate Care is seeking a compassionate Restorative Nurse (RN or LPN) to join our team in delivering restorative care and rehabilitation. You will develop personalized care plans to help residents reach their highest independence levels and work closely with nursing and therapy teams for coordinated care.

Qualifications

Responsibilities

• Develop and implement Restorative Care Plans to promote resident independence.
• Monitor and evaluate resident progress; adjust care plans as needed.
• Provide therapeutic activities including exercises and mobility support.
• Educate residents and families on exercises, safety, and recovery strategies.
• Collaborate with Nursing and Therapy teams to ensure coordinated care.
• Document and record resident progress accurately.

Your Responsibilities:

• Develop & Implement Restorative Care Plans – Work with residents and the healthcare team to promote functional independence.
• Monitor & Evaluate Resident Progress – Track the effectiveness of restorative interventions and adjust care plans as needed.
• Provide Therapeutic Activities – Assist residents with exercises, mobility, and functional tasks to maintain or improve their abilities.
• Educate Residents & Families – Provide guidance on exercises, safety, and strategies to enhance recovery.
• Collaborate with Nursing & Therapy Teams – Ensure coordinated care across all disciplines.
• Document & Record Resident Progress – Maintain accurate and up-to-date records of care and progress.

Qualifications:

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) License
• Experience in Restorative Nursing, Rehab, or Long-Term Care (Preferred).
