Job summary

Clinical & Support Options, Inc. is seeking a Transitional Services Outreach Counselor for the Respite program in Pittsfield, MA.

This role focuses on short-term case coordination, outreach, and transition planning to support individuals moving from respite beds to permanent living arrangements. You will assess living skills, document transition plans, assist with advocacy, coordinate entitlements, and provide intensive support during crises or transitions, while helping clients access housing,

Qualifications

• Bachelor's degree in a mental health discipline plus two years' community mental health experience, or an Associate's degree with two years' experience, or a high school diploma with five years' related experience.
• Valid driver's license and reliable transportation for outreach and community activities.

Responsibilities

• Provide assessment of independent living skills and skills training in areas such as transportation, money management, cleaning, laundry, food preparation, personal grooming and health care.
• Document a transition plan based on assessed needs and transition goals.
• Assist with advocacy and mediation as appropriate.
• Identify and obtain entitlements including housing and tenancy supports.
• Provide intensive support during psychiatric crises or transitions.
• Assist with insurance and benefit applications and accessing services.
• Help develop a natural support network of family and friends.
• Support negotiation of relationships with roommates, landlords and others.
• Coordinate meaningful day activities including clubhouse participation, schooling, employment or volunteering.
• Identify recreational and social activities and provide transportation as needed.
• Maintain contacts with family, DMH case managers, therapists and others to support clients.
