Job summary

UCP of Western Massachusetts is seeking a dedicated Community Living Options Coordinator in Pittsfield, MA to support individuals moving from nursing homes to community living. You will develop and finalize Transitional Assistance Plans and Housing Search Entity Plans, working under the Assistant Director to enable successful transitions.

Key duties include collaboration with participants, staff, and community resources, plus handling administrative tasks to keep the Community Living Options

Qualifications

• Experience in housing, community living coordination, case management, or related fields preferred.
• Strong organizational and administrative skills.
• Ability to work collaboratively with participants, staff, and community partners.
• Excellent communication and interpersonal skills.
• Commitment to supporting individuals in achieving independence and quality community living.
• Ability to manage multiple tasks and prioritize effectively.

Responsibilities

• Develop and finalize Transitional Assistance Plans and Housing Search Entity Plans.
• Collaborate with participants and under supervision to facilitate transitions.
• Assist participants in transitioning from nursing homes to community living environments.
• Provide direct support and engage with community resources to ensure smooth transitions.
• Complete administrative tasks for the Community Living Options department and support overall agency operations.
• Represent the agency positively to members, staff, and visitors through professional interactions.
