Qualifications

• Bachelor’s degree in social work, human services, or related field.
• 3+ years of healthcare or disability services experience.
• 2+ years of supervisory experience.
• Experience coordinating care in regulated settings.
• Strong leadership, communication, organization, and collaboration.

Responsibilities

• Lead and develop the Respite Care Coordination Team.
• Oversee client intake, eligibility determinations, ISPs, and care documentation.
• Oversee scheduling and placement to support program capacity and goals.
• Ensure compliance with licensing, Medicaid, and policies.
• Collaborate across departments to develop scalable processes.
