Graceworks Lutheran Services is seeking a Respite Care Coordinator in Washington to support patients in navigating healthcare systems and linking them to community resources. The Respite Care Coordinator plays a critical role in supporting respite patients by navigating healthcare systems, coordinating care, and linking them to community resources to promote better health outcomes. They assist with enrollment, eligibility, patient engagement, and social service referrals while ensuring respite patients receive appropriate follow-up care. This role requires living on site.
Supervisory Responsibilities: This position has no direct reports.

With multiple locations and settings, PCHS’ work environment and physical demands vary greatly depending on the employee’s role. Essential job functions include the following: The employee is required to talk, hear, see, smell, sit and stand.
