Qualifications

• Minimum 2 years in a non-profit or community health clinic.
• Knowledge of disease management concepts, health messaging, and clinical guidelines for respiratory health.
• Passion for eliminating health disparities and empowering underserved communities.
• Flexibility to work weekend schedules for outreach events.

Responsibilities

• Track program and contract deliverables tied to respiratory health initiatives.
• Lead weekend outreach events, coordinating logistics and health education/screening.
• Coordinate activities across navigation, outreach, and patient services teams.
• Engage stakeholders across the organization, partners, and funders.
• Oversee recruitment, enrollment, and outreach for respiratory health resources.
• Ensure documentation of all program activities and report data issues.
• Keep clinic and navigation staff informed on workflows and referrals.
• Draft protocols, training modules, and culturally tailored communications.
• Identify training gaps and contribute to program improvement.
