Position Summary:

The Clinical Programs Coordinator plays a vital role in the oversight, coordination, and implementation of Roots Community Health Center's Respiratory Health clinical initiatives. This position focuses on driving health equity, reducing health disparities, and delivering targeted education, screening, and navigation services for respiratory health and infectious disease management. In addition to clinic program operations, this role is instrumental in extending critical respiratory health resources directly into the community.

Requirements

Essential Functions:

  • Program Execution & Deliverables: Track program and contract deliverables specifically tied to respiratory health initiatives((i.e., Asthma and COVID), ensuring all targets are met in a timely and effective manner.
  • Community & Weekend Outreach: Serve as an instrumental lead for weekend outreach events, coordinating logistics, participant engagement, and field-based health education/screening efforts.
  • Team Coordination: Coordinate activities across patient navigation, outreach, and patient services teams to ensure seamless service delivery.
  • Stakeholder Engagement: Communicate effectively across all levels of the organization, community partners, and funders to advance respiratory health program goals.
  • Patient Navigation & Outreach: Oversee recruitment, enrollment, and targeted outreach to maximize community participation in respiratory health prevention/treatment resources.
  • Documentation & Data Quality: Ensure thorough and consistent documentation of all program activities, immediately reporting data collection issues to the Program Administrator and Medical Director.
  • Clinic Linkage & Communication: Keep clinic and navigation staff informed on program workflows, referral criteria, and continuum-of-care linkages for respiratory health patients.
  • Materials & Protocol Development: Assist in drafting protocols, training modules, and culturally tailored health communications regarding respiratory care and infection control.
  • Quality Improvement: Identify training gaps and operational bottlenecks, contributing to clinical strategy and program innovation.

Preferred Qualifications & Work Experience

  • Minimum of 2 years of experience working in a non-profit organization or community health clinic.
  • Knowledge of, or willingness to learn, basic disease management concepts, health messaging, and clinical guidelines specific to respiratory health.
  • Demonstrated passion for eliminating health disparities and empowering underserved communities.
  • Flexibility to work weekend schedules for outreach events as required.

  • Technical Adaptability: Proficiency with Microsoft Office, G-Suite, and the ability to quickly learn Electronic Medical Record (EMR) systems and specialized databases.
  • Collaborative Problem-Solving: Ability to identify gaps in care or communication and work proactively with multidisciplinary teams to resolve issues.
  • Driver’s License: Valid California Driver’s License with a safe driving record (or ability to obtain).
Physical Demands & Working Conditions
  • Frequent standing, walking, stair climbing, bending, stooping, and lifting up to 25 lbs.
  • Gross and fine manual dexterity to operate standard office equipment and keyboards.
  • Ability to work both indoors within a clinic environment and outdoors during street-based outreach or community events.
